These Terms of Service (“Terms”) govern your access to and use of MD Concierge Health’s website, services, and membership programs (collectively, the “Services”). By using our Services, you acknowledge that you have read, accepted, and agreed to be bound by these Terms.

1. Acceptance of Terms

These Terms form a legally binding agreement between you and MD Concierge Health. By using the Services, you agree to comply with these Terms, which may be updated from time to time. Updates will be effective upon posting to our website. Continued use of the Services after changes are posted constitutes acceptance of those changes.

2. User Accounts

Certain features may require creating a user account. You are responsible for:

• Providing accurate and up-to-date information.

• Maintaining confidentiality of your login credentials.

• Promptly notifying us of any unauthorized use of your account.

MD Concierge Health is not responsible for loss or damages arising from failure to safeguard your credentials.

3. Use of Services

We grant you a limited, non-exclusive license to use our Services for personal, non-commercial purposes. You agree not to:

• Reproduce, distribute, or exploit the Services without written permission.

• Engage in scraping, hacking, or unauthorized data collection.

• Violate intellectual property rights or applicable law.

4. Proprietary Rights

All content, software, and materials are the property of MD Concierge Health or its licensors, protected under U.S. and international intellectual property laws.

5. Medical Disclaimer

​

Our Services facilitate access to healthcare through a licensed physician. MD Concierge Health does not replace the need for emergency services. If you experience a medical emergency, call 911 immediately.

6. Membership Services

​

Membership plans are structured as follows:

​

• Vital Membership – Core concierge services and wellness management.

• Thrive Membership – Expanded access and advanced health optimization services.

• Elite Home Membership – Premier access, including home visits and priority scheduling.

Certain services may be offered as add-ons outside the membership tiers, including but not limited to IV therapy, advanced lab testing, prescription management, and subscription-based medications.

7. Meet & Greet Consultations

​

We offer a one-time “Meet & Greet” appointment to allow prospective members to learn about our services. This meeting is not a trial membership and does not establish an ongoing patient-physician relationship unless you enroll in a membership.

8. Payments & Membership Terms

​

• Billing: Membership fees are billed monthly or annually, depending on the plan selected.

• Commitment: All memberships require a minimum commitment of three (3) months.

• Renewal: After the 3-month commitment, memberships automatically continue month-to-month unless canceled with at least 30 days’ written notice.

• Payment Methods: We accept payments via secure online payment processing. By enrolling, you authorize MD Concierge Health to charge your chosen payment method automatically for recurring membership fees and any applicable add-on services.

9. Cancellations & Refunds

• Appointments: Cancellations require at least 24 hours’ notice. No-shows or late cancellations may be billed in full.

• Memberships: Early termination before the 3-month commitment period requires full payment of the remaining term.

• Products/Pharmacy Items: No refunds are offered once dispensed (e.g., peptides, IV medications, hormone therapies).

10. Indemnification

You agree to indemnify and hold harmless MD Concierge Health, its staff, and affiliates from claims, damages, or expenses arising from your use of the Services or violation of these Terms.

11. Third-Party Services

​

Our Services may link to third-party providers (e.g., labs, pharmacies). MD Concierge Health is not responsible for third-party services, billing, or policies.

12. Disclaimers & Limitation of Liability

​

Services are provided “as is.”

​

• No warranties are made regarding uninterrupted or error-free operation.

• Liability for any claim will not exceed the amount you paid for the Services.

• We are not liable for indirect, incidental, or consequential damages.

13. Termination

​

Either party may terminate Services at any time, subject to membership commitments. MD Concierge Health may suspend or terminate access if Terms are violated.

14. Governing Law

​

These Terms are governed by the laws of California. Any disputes shall be resolved in the courts of Los Angeles County, California. Jury trial rights are waived.
